Eligibility: Graduation or Equivalent or minimum 2 years of relevant experience (experience-based learners can directly enroll and certify).

Objective: The Certified Incident Investigation Officer course is designed to equip participants with the skills and knowledge required to systematically investigate workplace incidents, including accidents, near misses, and unsafe conditions. The course covers incident classification, immediate and root cause analysis (using methods such as the 5 Whys and Fishbone Diagram), evidence gathering, interviewing techniques, incident documentation, and legal and regulatory considerations. 

Certification: Within 5 days after Completion of Online Assessment.

Steps to become a GSDCI Certified Professional:

Step 1: Select your certification you want to pursue.

Step 2: Click on get certified tab, new pop up window will open.

Step 3: Click on pay certification fee, you will be redirected to billing details page.

Step 4: Fill your details and click on pay certification fee, You will be redirected to payment gateway, pay fee by any available options like Card(Debit/Credit), Wallet, Paytm, Net banking, UPI and Google pay.

Step 5: You will get Login Credentials of Online E-Books and Online assessment link on your email id, within 48 hrs of payment.

Step 6: After completion of online assessment, you can download your Certificate Immediately.

Assessment Modules:

Module 1: Introduction to Incident Investigation and Legal Framework: Definition and types of incidents (accidents, near misses, dangerous occurrences), Importance and objectives of incident investigation, Roles and responsibilities of an Incident Investigation Officer, Legal and regulatory requirements (OSHA, ISO 45001, local laws), Confidentiality and ethics in investigation procedures, Integration of investigations within the safety management system.

Module 2: Preparation and Immediate Response to Incidents: Initial actions at the incident scene: securing the area and ensuring safety, Roles of first responders and communication protocols, Preserving evidence and protecting involved personnel, Notifying relevant authorities and internal stakeholders, Developing an incident response checklist, Collecting initial data for investigation launch.

Module 3: Data Collection and Evidence Gathering Techniques: Conducting interviews with witnesses and involved personnel, Observational techniques and environmental assessments, Collecting physical evidence and taking photographs, Reviewing documents: permits, maintenance logs, SOPs, Incident timeline reconstruction methods, Ensuring objectivity and accuracy in information gathering.

Module 4: Root Cause Analysis and Investigation Tools: Differentiating between immediate, underlying, and root causes, Tools and techniques: 5 Whys, Fishbone Diagram, Fault Tree Analysis, Event and Causal Factor Charting (ECFC), Human factors and system failures analysis, Determining causation patterns and contributing elements.

Module 5: Report Writing and Corrective Action Planning: Structure and components of an incident investigation report, Writing clear, concise, and fact-based conclusions, Identifying and recommending corrective and preventive actions (CAPA), Assigning responsibilities and timelines for action items, Communicating findings to management and workers, Developing reports for legal or regulatory submission.

Module 6: Follow-Up, Review, and Continuous Improvement: Monitoring implementation of corrective actions, Evaluating the effectiveness of preventive measures, Feeding investigation findings into training and risk assessments, Updating procedures, policies, and safety systems, Learning from incidents to avoid recurrence, Promoting a culture of transparency and learning through incident analysis.

  • Exam Type: Multiple Choice Questions.
  • Maximum Marks- 100, Passing Marks- 50%.
